This policy describes how Nexoform, Inc., a Delaware corporation (“Nexoform”, “we”, “us”), handles personal information collected through this website, nexoformai.com. It does not cover the Nexoform product itself: use of the product is governed by the written agreements between Nexoform and each customer practice, which take precedence for anything processed inside the product.

This site collects very little. If you submit the demo request form, we receive the information you enter: your name, work email address, practice name, role, and any message you include. If you email us, we receive your correspondence.

Our hosting provider also keeps standard server logs (such as IP address, browser type, and pages requested) for the purpose of operating and securing the site.

We do not use advertising trackers, third-party analytics scripts, or tracking cookies on this site.

We use the information above to respond to your inquiry, schedule and conduct demos, communicate with you about Nexoform, and operate and secure this website. We do not sell personal information, and we do not share it with third parties for their own marketing or advertising.

We rely on a small number of service providers to run this site and our business, such as our website hosting provider and our email provider. These providers process information on our behalf and are not permitted to use it for their own purposes.

This website is not intended to collect patient or health information, and we ask that you not include any in the demo form or in email to us.

Within the Nexoform product, protected health information is processed only on behalf of the medical practices that are our customers, under the agreements we sign with them, including business associate agreements as required by HIPAA. Nexoform acts as a business associate of its customer practices. If you are a patient of a practice that uses Nexoform, your relationship is with that practice: please direct any questions or requests about your health information to them.

We keep demo requests and correspondence for as long as needed to follow up and maintain our business records, and we delete or anonymize information when it is no longer needed. Server logs are retained for a limited period by our hosting provider.

You may ask us to access, correct, or delete the personal information we hold about you from this website, or to stop contacting you, by emailing info@nexoformai.com. Depending on where you live, you may have additional rights under laws such as the California Consumer Privacy Act or the EU General Data Protection Regulation; we will honor requests as those laws require. We will not treat you differently for exercising these rights.

This site is intended for professionals at medical practices and is not directed to children. We do not knowingly collect personal information from children through this site.

If we change this policy, we will post the updated version here and revise the date at the top of the page.
